WASHINGTON — The Food and Drug Administration has quietly pushed back its deadline to ban the use of electrical stimulation devices on patients, cautioning Thursday that more peer-reviewed data is needed before definitively linking forced electrocution to negative health outcomes.
The ban, originally slated to take effect later this year, targets graduated electronic decelerators—devices physically attached to patients' bodies to deliver painful shocks as behavioral correction. However, FDA regulators noted in a preliminary guidance document that rushing a federal prohibition without a robust, multi-year cohort study might be premature.
A recent retrospective analysis published in a JAMA paper suggested a strong association between the delivery of 45-milliamp cutaneous shocks and sudden, localized tissue damage. Nevertheless, agency officials warned that the sample size remains limited, and the existing data relies heavily on self-reported expressions of agony, which can introduce significant methodological bias.
While laymen might observe a patient screaming and assume the electric shock is the primary risk factor, the clinical picture is rarely that simple. We simply do not have the statistical power to say whether a sudden cessation of high-voltage discipline wouldn't carry its own unstudied side effects.
The regulatory delay provides a temporary reprieve for the single Massachusetts facility that currently includes the devices in its care pathway. Experts caution that an abrupt withdrawal of the shocks could trigger an unpredictable disruption in baseline suffering, recommending instead a phased step-down protocol where patients are slowly transitioned to standard household voltage to mitigate withdrawal risks.
Until further research is conducted, the CDC advises that any ongoing electrocution should be administered in a well-ventilated clinical setting, adding that the current guidance does not constitute an endorsement of shocking patients, only an acknowledgement that the literature has not yet definitively ruled it out.
